Personalized care

The practice is deliberately small. When we take on a patient, that patient receives Dr. Giddens's personal attention and consistent communication: the same physician at every visit, a clear plan in writing, and a reply to messages within one business day.

No two people arrive with the same history, and no two treatment plans here look alike. Personalized care means the plan is built around you rather than around a diagnosis:

We start with your story
The first visit is long enough (60 to 90 minutes) to hear what brought you in, what has and hasn't helped before, how you sleep, what you drink or use, what your days look like, and what you want to be different. For children and adolescents, parents, and with permission teachers and therapists, add their view.
Diagnosis is a starting point, not a label
Two people with "depression" may need very different things: one has untreated sleep apnea, another is drinking to fall asleep, a third has ADHD that was never recognized. We look for what is driving the symptoms, not only what to call them.
Treatment fits your life
Medication choices account for your other medical conditions, your schedule, your preferences about side effects, and what you are willing to take. Therapy recommendations match what you have time and appetite for. Nothing is prescribed by default.
We measure, then adjust
Between visits you complete brief rating scales, so changes are based on how you are actually doing, not on a single conversation. If something is not working, we change it rather than wait.
Consistent communication
You will know what the plan is, why, and what comes next. Messages are answered within one business day, results and changes are explained rather than announced, and you leave each visit with a written summary.
One doctor, both cities
Whether you are seen in New York, in Washington, DC, or by video, you see the same physician who knows your history, and your care coordinates with your therapist, primary care doctor, pediatrician, or school.
